What is Oculoplasty?
Kısa Yanıt
Oculoplasty (oculoplastic surgery) is a subspecialty of ophthalmology that deals with the surgical management of the eyelids, periocular soft tissues, tear drainage system, and the orbit. It encompasses both the treatment of functional disorders and the performance of aesthetic procedures in the periocular region. Surgeons practising in this field are ophthalmologists who have undertaken additional training in periocular surgery.
Definition of Oculoplastic Surgery
The term oculoplastic surgery derives from the Latin oculus (eye) and the Greek plastikos (moulding, repair). As a subdivision of ophthalmology, it concerns itself with the surgical treatment of structures that lie outside the globe itself but are directly related to ocular function.
Oculoplastic surgery serves three principal purposes:
- Functional correction: Treating eyelid drooping, lid malpositions, and tear system obstructions to preserve ocular health and visual comfort.
- Reconstruction: Restoring periocular structures following tumour excision, trauma, or congenital anomalies.
- Aesthetic improvement: Addressing eyelid laxity, under-eye bags, and age-related changes in the periocular area.
Oculoplastic Surgery vs. Plastic Surgery
Both disciplines employ surgical techniques, but their foundational training and areas of focus differ. This distinction has a direct bearing on the approach to periocular conditions.
| Criterion | Oculoplastic Surgeon | Plastic Surgeon |
|---|---|---|
| Core specialty | Ophthalmology | General surgery or plastic surgery |
| Focus area | Eyelids, periocular region, tear system, orbit only | Entire body (face, breast, abdomen, etc.) |
| Ocular anatomy knowledge | Advanced, including globe, cornea, and retina | General level |
| Ocular safety priority | Visual function and ocular surface protection are primary | Aesthetic outcome is primary focus |
| Tear system expertise | Diagnosis and surgical management of tear drainage disorders | Generally outside scope |
Areas Covered by Oculoplastic Surgery
Oculoplastic surgery encompasses a wide range of conditions affecting the periocular region:
- •Eyelid aesthetics: Upper blepharoplasty, lower blepharoplasty, and brow lift
- •Eyelid malpositions: Ptosis, entropion, ectropion
- •Eyelid lesions and tumours: Benign and malignant eyelid masses, xanthelasma
- •Tear drainage disorders: Nasolacrimal duct obstruction, dacryocystorhinostomy (DCR)
- •Thyroid-related lid changes: Eyelid retraction and proptosis in Graves disease
- •Periocular reconstruction: Tissue repair following tumour removal or trauma
When Should You Consult an Oculoplastic Surgeon?
You may wish to consult an oculoplastic surgeon if you experience any of the following:
- •Drooping upper eyelids that interfere with vision or cause fatigue
- •Eyelids turning inward or outward, causing irritation or tearing
- •Persistent watering of the eyes (epiphora)
- •A lump, growth, or colour change on or around the eyelids
- •Excess eyelid skin, under-eye bags, or age-related periocular changes
- •Eyelid changes associated with thyroid disease
The Examination Process
During an oculoplastic consultation, the eyelids, periocular tissues, and tear drainage system are examined in detail. Clinical measurements are taken, including eyelid position, skin elasticity, and muscle function. A tear drainage test may be performed. Where indicated, photographs are recorded and imaging studies may be requested.
Following the examination, the findings are discussed, and the available treatment options — surgical and non-surgical — are explained clearly.
Treatment Options
Treatment depends on the underlying diagnosis and the individual patient's anatomy, expectations, and general health. Options may include:
- •Observation and follow-up: When the condition is mild and does not yet warrant intervention.
- •Medical management: Eye drops, ointments, or systemic treatment for certain inflammatory or infectious conditions.
- •Surgical intervention: Blepharoplasty, ptosis repair, lid malposition correction, tumour excision with reconstruction, or tear drainage surgery, depending on the diagnosis.
All treatment decisions are made after a thorough clinical evaluation and discussion with the patient.
Clinical Note
“In oculoplastic surgery, every millimetre matters. The eyelids are among the thinnest and most delicate tissues in the body. A procedure must not only look right but must also preserve the eyelid's ability to protect the cornea, spread the tear film, and allow a full field of vision.”
