I Found a Lump on My Eyelid -- What Should I Do?

First Step: Do Not Panic

Noticing a lump or swelling on the eyelid can be concerning. However, the vast majority of eyelid lesions are benign. That said, any new or changing eyelid lump should be evaluated by an appropriate specialist to confirm its nature and determine whether treatment is needed.

Most Common Benign Causes

The following are the most frequently encountered benign eyelid lumps:

  • Chalazion: A painless, firm nodule caused by blockage of a meibomian (oil) gland. It is the single most common eyelid lump. Warm compresses often help, but persistent chalazia may require incision and drainage.
  • Hordeolum (stye): An acute, painful, red swelling caused by bacterial infection of a gland at the lash root or inside the lid. Most styes resolve with warm compresses and topical antibiotics.
  • Papilloma: A small, flesh-coloured, pedunculated growth related to skin aging or viral infection (HPV). It is harmless and can be excised if bothersome.
  • Cyst (sudoriferous or sebaceous): Fluid-filled or keratin-filled sacs that grow slowly and are entirely benign.

Warning Signs That Warrant Prompt Evaluation

While most eyelid lumps are benign, certain features should raise concern for a potentially malignant lesion. The following signs require prompt specialist assessment:

  • Progressive, painless growth over weeks to months
  • Loss of eyelashes (madarosis) in the area of the lesion
  • Distortion of the eyelid margin architecture
  • Ulceration, bleeding, or crusting of the lesion surface
  • A pearly, rolled border (characteristic of basal cell carcinoma)
  • Firm, fixed mass that does not move freely
  • A recurrent chalazion in the same location

Malignant Eyelid Lesions

The most common malignant eyelid tumour is basal cell carcinoma, which is locally invasive but rarely metastasises. Squamous cell carcinoma and sebaceous gland carcinoma are less common but more aggressive. Early detection and complete surgical excision are the cornerstones of treatment and offer excellent prognosis when the lesion is caught early.

Eyelid lumps are best evaluated by an ophthalmologist with experience in oculoplastic surgery. This specialist can perform a thorough clinical examination, differentiate benign from suspicious lesions, and carry out excision or biopsy with attention to eyelid function and cosmesis.
